Ultrasound and Echocardiography Mentorship Program Agreement

Welcome to the Ultrasound and Echocardiography Mentorship Program by Saving Lives Academy (SLA). This formal agreement outlines the structure, terms, and responsibilities between SLA and the participating Mentee (Trainee) enrolled in our educational mentorship pathway.

Whether you’re a medical professional pursuing FUSIC, EDEC, or National Board of Echocardiograph, this agreement ensures a safe, structured, and compliant learning environment.

1. Parties Involved

This Mentorship Agreement is entered into by and between:

   • Saving Lives Academy (SLA) – a trusted platform for clinical ultrasound and echocardiography training.

   • The trainee – a medical professional, seeking training under the guidance of an accredited Mentor.

2. Purpose of the Agreement

   SLA provides secure, GDPR-compliant access to training content, including ultrasound/echocardiography image/video review, feedback, and guided mentorship.

   The Mentee participates solely for educational and accreditation purposes, with no clinical or diagnostic relationship established between mentor and trainee.

3. Scope of Mentorship and Responsibilities

   For the Mentee (Trainee):

   ✅ Use all training content strictly for learning and accreditation.

   🚫 Understand that mentorship is not a substitute for clinical diagnosis or consultation.

   🏥 Any clinical concerns identified during scanning must be escalated through your hospital’s formal channels.

   📄 Do not include any mentor comments in patient records unless accredited and approved.

   🙋‍♀️ Confirm that valid patient consent has been obtained for any uploaded scans or training materials.

4. Data Privacy & GDPR Compliance

   SLA’s platform adheres fully to UK GDPR regulations.

    ❌ Do not upload patient-identifiable information under any circumstance.

    ✅ All case submissions must remain anonymised and used solely for training.

5. Legal Disclaimer & Liability

   • SLA and affiliated Mentors bear no liability for clinical decisions or actions based on training materials.

   • Any breach of agreement by the trainee is their sole responsibility.

6. Duration and Termination

   This agreement remains valid from the Effective Date for one year or until terminated by either party with 30 days’ written notice.

7. Governing Law

   This agreement is governed by the laws of the United Kingdom.

   Any dispute will be resolved through arbitration in the UK.

8. Entire Agreement

   This document represents the entire agreement between the trainee and Saving Lives Academy and supersedes all prior verbal or written agreements related to this program.
